Telephone: Accepted

Know the following when requesting
   * Name
   * Address

Permanent: Application must be completed and notarized within 30 days of submission. When the file is 100 percent, it is then reviewed by the technician. Upon approval, a letter goes to the applicant requesting the endorsement fee and registration fee. When the fee is received at the Board's office, number is issued. If you need to get a number before the next Board meeting, the physician can request an Interview. The Board will NEVER offer an interview. The number is issued within one to five days of the interview.

Fees:
Permanent: $325 application fee +$225 endorsement fee +$170 or $340 registration fee
Board Meetings Schedule: Once a month, usually the second Wednesday
Pending Malpractice: If a doctor has malpractice, the file will go to malpractice review and, depending on the how busy the Board is, will probably stay there for six to eight weeks. Then the malpractice review committee will determine whether or not it needs to go to a Board meeting. If it doesn't, then it goes straight to credentialing where it will probably stay for another six to eight weeks.
