Medical School: Graduate of an AMA/AOA-approved medical/osteopathic school
Post-graduate Training: One year AMA/AOA/ACGME-approved training in the U.S. or Canada
National Boards: Accepted
Flex: Accepted. Distinction between old and new exams: Old exams: Must have obtained a weighted average of 75 of better. New exams: Must have obtained a score of 75 or better on each component. From the time applicant sits for Part 1 until the time he/she passes all parts cannot exceed seven years.
LMCC: Accepted
State Exam: Prior to June 1979 Florida and Puerto Rico not accepted.
USMLE: Accepted
10-YEAR RULE: Yes. Certification or re-certification in last 10 years will exempt applicant.

Know the following when requesting-

   * Medical school and date of graduation
   * Current state of licensure
   * Board certification and date (if applicable)

Permanent:
When the file is 100 percent, the file must go to a Board meeting and Licensing Committee meeting. Deadline to have file completed is the first Wednesday of the month. The Board meets on the third Wednesday, and the Licensing Committee meets on the fourth Wednesday. Number will be issued the day the Licensing Committee meets, provided that the registration fee has been paid. Applicant can obtain license number by telephone after 2 p.m. An interview may be required if there is derogatory information. Effective February 1996, Board will only speak with applicant.

Permanent: $175 + $75 on completion. $250 total.
Board Meetings Schedule: Third Wednesday of every month.
Pending Malpractice: This is a Board decision. Must apply.
